CARE AND PATHWAY REVIEW
SEMI - INDEPENDENT LIVING
We are all entitled to our independence and wellbeing. A reason why we encourage individuals with everyday tasks, including personal care, housing related tasks, household chores, grocery shopping and managing their money.
We do ensure that every independent person’s needs is fulfilled with credible expectations to succeed for great ambitions and career path.
Level of Support
ALL YOUNG PEOPLE WILL RECEIVE SUPPORT WITH:
Care and Pathway review
We liaise with the social worker, family therapist, psychologist, parents and representatives from all relevant multi agency teams. The team lead would be updated on the young person’s placement plan as well as any work that has been undertaken in supporting the placement of the young person.
Our Care features
Workshops
We support our young people to identify their skills, abilities and behaviour that will enhance their life chances and equip them to be responsible adults and effective citizens.
Personal care
We support them to carry out tasks appropriate for their age independently in order to develop their skills, e.g., shopping, cooking, washing and ironing. Young people may be supervised if required. The task is increased gradually depending on the outcome.
Health and Safety
Divine Dees Care has a comprehensive health and safety policy outlining responsibilities and actions required to promote the health and safety of young people, staff and visitors to the home. Ongoing risk assessment of our Young people are carried out in order to protect them from physical, emotional, sexual, financial, discriminatory, organisational and institution abuse. All abuse will be reported in accordance with the Local safeguarding children Board.
Learning
We understand the importance of developments of our young people. In order to achieve this we organise coaching and mentoring on one to one basis as well as group sessions. Through this coaching and mentoring, we are able to teach them other skills such as team work, integrity, positive attitude, self-management and resilience, This may result in pursuing further education through other avenues or engaging in a training programme, apprenticeship or employment. All young people have a Personal Education Plan, which includes information about their career aspirations and ambitions.
Support
Supporting our Young people is completely dependent on the needs of each individual.
We offer a flexible and person-centred approach to allow us to align our services to meet the unique needs of the young people we support. We will ensure right from the beginning that our Young person know what we will do to support them and our responsibilities to them.
Activities
Young people are encouraged to engage in recreational activities outside the home. This includes encouraging the young person to access sporting, leisure and cultural activities. Sport and physical activity are encouraged as part of the health promoting process to enable healthy living as well as positive peer group interaction.
